Air in the cooling system?
Stock PCM programming turns the fans on low speed at 226*F and high speed at 235*F. Do you have access to a scanner that would allow you to see what the coolant temp sensor is telling the PCM? A faulty sensor could prevent the fans from coming on at the correct temperature.
I don't have a scanner but was planning on getting one one day. Maybe I should just replace the sensor just in case. If i leave the heater on the fans stay on and the car stays cool. Maybe I should just invest in a manual fan switch and be done with the whole issue.
