Exhaust manifolds must be removed. They're best removed after the motor mounts, tranny, etc. are loose and the motor's on the cherry picker.
Buy a 'Happy Hooker' from TPIS (yes, that's the technical name); it will attatch the cherry picker to your intake manifold via the four surprisingly strong injector rail bolts.
I've removed the motor both with the compressor/radiator/ac condensor in and out; it's muuuch easier to remove with the radiator/compressor/radiator out, but then you have to charge the system.
