problem shifting/driving

Here are all the symptoms I seem to be having:

Sometimes my clutch will slip real bad under heavy load, but it doesn't happen often, and if it does happen it seems to correct itself after a little bit of driving more and always after a cool down. The clutch is still fairly new with a resurfaced flywheel. I'm thinking that maybe my hydraulics are bad?? Is that possible?

Also, lately it has become very hard to shift sometimes, more so in reverse. Actually today i was stopped completely and it grinded trying to shift in reverse. How can it grind if the car isn't moving and the clutch is disengaged? But sometimes at a light when i shift to 1st i have to put some muscle on it to shift. I replaced the tranny mount about 6 months ago...about the same time the clutch was done.

Could all of this be linked to the hydraulics? How hard is it to replaces the master/slave cylinder?
