Yeah, it was publicised pretty well when it happened, and I'm sure it is possible. Darwinism in action, definitely. You could just as easily ask if it's possible to set your car on fire in the garage from your fuel tank, or have it fall off a lift, or roll down the street, etc, etc. Bad things can and do happen.

Pressure up any bottle over capitcy, after jacking with it's safety relief, and it's going to spell trouble, whether its nitrous, CO2, oxygen, accetylene, you name it.
Typical... it's amazing how many people attempt to find a scapegoat for their own stupidity, instead of simply taking responsibility for their actions.

Actually, I would assume that the total burst psi on 2 caps would be higher than 3000psi when used together. It's my understanding that the caps psi rating is based on tensil strength from the center of the cap/disc. I believe that adding a senond cap would provide enough additional tensil strength to more than double the blow off psi that a single disc provides.
I remember seeing this when it first happened, I think it was attributed to not turning off the bottle heater and the extra blow off cap, as you have previously stated. Lesson: Dont modify with the safety equipment!!
