trans grinds when shifting.... What is it?

When I go to take off it grinds. If I give it more gas while letting the clutch out, it grinds worse. I have to let the clutch out really slow and leave it at like 1500-200 RPM. If I don't let out slow, it will grind. I am thinking t/o bearing. What else could it be? Clutch grabs good when I take off. It's like I have to pop the clutch. It's not chattering, it's a grind. Coming from the trans... I'm stumped.

your friction disc may have come apart, causing metal to metal contact.

that doesn't explain how it grabs so good once it's engaged though... I'm thinking I need a new clutch kit, but I can't figure out what would cause this. I've never seen anything like this before. Anyone else have a suggestion?
