I am not an expert on that particular swap but I know that the 95's usually have an odb2 looking port with the pins arranged differently. In my uncles shop he has several key chips that plug into the cable in-line, thus swapping the pins so that it can be read. I did see a schematic somewhere around showing the relationship.
The ICM washer trick may work a little but it is far far far better to remount it completely. I myself mounted mine on a computer heat sink with aluminum fins and thermal grease from STAPLES, removing it completely from the super hot head. Wow that was some foolish engineering. Now when the engine is operating the heat sink is cold to the touch, while you would not be able to hold your finger on the front of the head.
The lack of power when warm also sounds like fuel pump possibly.
