Need some help Car stumbles bad!

Well I just got my car back out after replacing the rear end and it runs like a bad of crap. First thing I did was connect it Free Scan to see if any codes were being thrown, Right 02. So I replaced the O2 and bought proper extentions for the headers. Now in the scan the code is gone and hasn't come back but the Right O2 readings sit at 460mV or so.

I checked all the wires coming from the O2 to the pcm for continuity all is good and I've even switched out the PCM.

Any other ideas? Would a header or vacuum leak do this? What vacuum lines should I check?

Dirty Injectors? Would it help if I posted a datascan?

Well I've got a new plan I'm gonna replace all 4 wires between the two connectors (PCM and O2). Any advice on how to do ths?
