shop did the work. how could i tell if it was off one.

You would have to take the timing cover off and look. Id just ask the shop what they did. It should be put on straight up or both dots on the gears should be at 12 o'clock. I dont see how a decent shop could get it wrong but they may have tried to advance the cam a couple of degrees like the old school builders liked to do.

the guy that did the work is well respected in the area for what he does. im 99.9% sure he lined the dots. would a larger cam need a retune?

Yes however your problem is not the cam.. its hte motor so figure out what is wrong with the current setup and then youll be happy. I wouldnt rule out the heads either since you changed those also.
