Datalog: Bad O2 sensor or Misfire?

Its been a little while now sence I fixed all the exhaust leaks and got the tune adjusted for changes so I figure the computer should have had time to learn its long term values, time to see whats up. I knew something was up before because my butt-o-meter was only reading about 240hp not the 275ish it whould. This datalog is kinda long sorry, but it is going 4 miles up the intersate, turn around a little WOT down the on ramp then 4 miles back and the drive from the interstate back to the shop.

Here are the Record numbers of interest:
194 - 238 WOT run upto Highway speed
1408 - 1432 2nd gear WOT
1432 - 1474 3rd gear WOT
1474 - 1487 4th gear WOT
After the highway runs there is a horrible bank to bank mismatch, most notibly 2860 to 2893. (6th gear WOT @ ~2200rpm)
Any input would be highly apreciated, thank you.

Ok, I've finally got some free time to look into this some more today. The mis balance has gotten far worse, I did another datalog and had +25% LT and +52% ST on the one side. There are no exhaust leaks on that side either. Any ideas?

EDIT O2 sensor was loose. And while under the car i noticed the header flange gasket on the one side is gone.
