Angry bent pushrod

Well we were tuning my car... And on the Tech 2 was showing that cylinder #1 was having a misfire.. and it kept fouling out plugs... well drove it down the road and found out.... Both the rocker arms where off cylinder 1 and one was bent like a shallow S and the other had a wobble...!

What could of caused this?

Cam Timming?

Valve smack the piston?

It showed error code 19

I replaced that injector drove it down the road and this happend
