balancer bolt

i was changing my opti and noticed that the main bolt holding the balancer on had came loose again.. it was loose when i first got the car, and i torqued it back down, and now it has worked its way loose again...

if it helps i have been changing some very nasty misses in the engine, and it completely dying it was getting so rough, would this cause it to work its way loose, or is there possibly something else bigger wrong with the engine??

oh and when it would run right for me it was smooth as could be all the way through the rpm range, so i dont think the engine is out of balance..

Does the bolt have the required thick washer under the bolt head? Is there any evidence that the damper hub is loose on the crank snout?
