Any possibility a prior owner installed a fan switch or wired them to be always-on?

Does your check engine light work?

I'd put a scanner on it to see if the fans are being commanded on or there are any possible trouble codes.

Yes, there are some, but not sure if they turn the fans on. That is why I suggest the scanner for fan command status. You have to start ruling things out somewhere...

The right scanner can do more than provide codes. It can read the various sensors, and the calculated engine operating parameters, allowing you to look at whether the data coming into the PCM from the sensors is correct, and whether the PCM is producing the required responses to the data.

That may be "normal" fan operation, the stock programming doesn't turn on the primary fan until somewhere around 225 degrees, if I recall. You could try turning on the A/C, to see if the primary (LH) fan turns on.
