Weird Clutch Failure

Weird Clutch Failure

In all the years I have worked on cars I have never seen a failure like this. The throwout bearing flange broke off while pulling in to a parking space!
Any one ever hear of this happening before?

Ended up destroying the clutch slave cylinder and pressure plate when the clutch fork was allowed to travel too far and contact the edge of the pressure plate. Replaced the clutch, pressure plate, throwout bearing, pilot bearing, rear main seal and resurfaced the flywheel on a car that just had the clutch replaced a few months ago.
