reverse is messed up!

reverse is messed up!
when i go to put the car in gear it grinds and my speedometer goes up to about 7-10 mph then i force it harder and it clicks into gearit also slips outta reverse sometimes...ne ideas on what this could be...possibly a bent fork any help would greatly be appreciated
