Broken Slave cylinder

Broken Slave cylinder

Well, I just got done replacing my clutch, everything went good, but i think i toasted the slave cylinder. When i got done and went to start the car the clutch pedal was a little hard then went right to the floor. The car started but the clutch wont disengage now. I checked under the car and the slave is leaking.

So first, does anyone have the part number for the complete clutch hydraulic assembly for a 95 z28? I ran a search but couldnt find anything useful.

And second, so i dont break a new slave, is there any way to test that the fork is installed and the TO bearing is working right?
