Steps:
1. Go buy a set of four tires from the list of eligible tire lines before June 17.
2. Photocopy your invoice.
3. Print out the PDF above and fill it out.
4. Mail in your invoice copy and the PDF printout by July 31.
5. Get a cash card with $25 or $100 on it (depending on which tires you bought) in the mail 6-8 weeks later.

As far as I can tell, the cash card is like a debit card and can be used anywhere. And as far as I can tell, it doesn't matter where you buy your tires.

In addition to the cash card, you also get a free "limited-edition" original movie soundtrack to the new Pixar movie, "CARS". I don't know if the soundtrack is any good, but I don't really care -- I'm in it for the $100.
