Uneven rotor wear?

Uneven rotor wear?

I've had problems with squeeky brakes for a while. I was always suspecting my right rear wheel. I've machined the rear rotors on my car and added new pads and the problem went away for a few weeks.

Today I looked at the rotor and noticed that the wear on it was not even almost as if the brake pad was not making complete contact. What can cause this? Could one of the pistons be bad? I'm wondering if I'll have to completely replace the braking assembly...
