If you had not mentioned the part where you changed the oil and fuel filter one time and it was okay, I would say it is a bad lifter, broken valve spring or bent pushrod.
Easy enough to pull the valve covers for a quick inspect of the springs. Remove the rockers to inspect the pushrods while you are there and then try to physically push down on each pushrod to see if you can make any lifter collapse at all. They should not push in at all.
Back to the change oil and filter issue. possibly a bad injector, and maybe someone can swap a complete fuel rail for a quick test.

thanks that's some good info
a thought....
if it were something mechanically broke/bent like a spring or pushrod, it wouldn't be an intermittant "1 month runs fine, 3 days runs like crap"
would it
i'm still thinking its either a lifter or injector
we'll see what happens after the oil flush and plenty of injector cleaner
