What could cause a reverse lockout not to work?

What could cause a reverse lockout not to work?
Ever since the swap I havent been able to have the reverse lockout to work right. I checked the wiring and Im gettting a code saying there is a problem with the circuit. What else could cause this? A bad solenoid??
