It isn't an exact replacement, but the part number is 10105324. I got that number from here > Shoeboxhttp://shbox.com/1/1995_evap.jpg. What I did was I went to an Autozone and got a piece of rubber hosing that fit on the lines. It wider at one end, so I used a razor tomake it slightly wider. This is just a fix until I get the OEM replacement. Hopefully that will solve your problem.

not saying this is whats wrong with your car.. but mine was doing that right before my opti went out on me.

Well, replacing the cracked elbow fixed the issue for me. But, I've also read that some people had to replace the opti when they ran in to similar symptoms.

ah..the surge is now back i think i will try completely cleaning the iat / maf

I had the exact same problem on my SS, only mine happend right away at startup (but was intermittent). I cleaned the TB, replaced the plugs/wires and changed the fuel filter on mine and all that still did not fix it. Since then I have replaced the ECT sensor to the PCM, IAT, IAC, MAP, both O2 sensors and the TPS. I also replaced the elbow from the EVAP solenoid to the TB because it was cracked. All that didn't help much. Finally, I unplugged the MAF, and restarted the car. This seemed to stop the surging. So I tried cleaning the MAF and the problem remained. I broke down and ordered a new MAF and the problem hasn't returned since!

So I would suggest unplugging the MAF, then restarting the car to see if that helps the surge. Maybe you could swap out the MAF with a freinds to see if that helps...

hey guys sorry for bringing back the thread but my car does the same thing. i got a 94 Formula. when its cold its fine and runs strong, but after around half an hour of driving the car starts jumping. im at a stop light and i have to press the break hard or the car will jump forward. the rpm jumps up to about 1000 rpm. also when i accelerate it sputters and doesnt want to run unless i press the pedal harder. it mostly doesnt do this below 1500 rpm. and doesnt do it around 3000 rpm but btween 1500 and 2500 it doesnt want to run, and then up top around 4000 rpm it does the same again. i changed(well the mechanic) all the spark plugs wires, gas filter intake elbow was ripped got a new one. the guy changed for other little sensor next to the intake elbow, and car still does this. any ideas guys?

This happened to my Formula. I replaced the IAT. (Probably the sensor you're thinking of) seemed to help it out. I recently had my 13 year old fuel pump replaced with a RaceTronix Fuel System. I haven't noticed a surge since.

for all those who are having this problem you really should get the car scanned to see what's going on before throwing parts at it.
i had this problem awhile ago. got to the point where the engine would stall every time i came to a stop. had it scanned and found out the knock sensor was done. every time the engine would decelerate the knock sensor would go crazy and i'd get at least 12* of timing (sometimes it would spike to 20*) pulled. i replaced and it was fine.
i had another incident with this after i put my headers on. whenever i would disconnect my PCM i'd get the same problem but it would go away after the PCM had learned and all was right with the world.
perhaps this can be some useful info...

the solution

everybody should read THIS! The analysis was very thorough, and solved the surging issue. This process was done at a shop with a sophisticated scanner. The solution ended up being a spark boot from what I've read. Anybody tried this?
