Running rich and dies once warm

Ok so this has been an ongoing problem for a while. First time it happened i wass sitting in traffic at the woodward cruise and my car started backfiring, wouldnt take any throttle and died. I let it cool down and it started back up and ran fine. I thought maybe running untuned for so long idling it loaded up and cleared out once it sat a while. I replaced the opti again, no fix. Jus replaced the ignition controll module because i keep pulling a code for it, no fix. It is running so rich right now it smells like a jet ski. My scanner is reading lt fuel trim at 129 right about 139 to 141 and left o2 is 6-700's but right is mid 400's. New code its pulling is faulty right 02. Is this the problem?

Has the car been tuned for all the mods? O2 sensor working correctly is very important. The readings should be all over the place and between 0xx and 9xx. Running rich will ruin a motor very fast if real bad.

Nope hasnt been tuned. It never used to run like this tho. Today driving it, it drove fine and wasnt cutting out at low rpms. When it acts up you can tell all cylinders arent firing untill about half throttle then it will clear out and go.

I would replace the o2 sensor and see if that helps.
