The PCV tube connects back into the intake manifold at the front...underneath the throttle body.

How many miles do you have on your car?

What mods have you done to:

The engine?

The exhaust?

Any changes to any valves?

Are the IAC/EGR valves factory, or have you replaced them recently?

How many miles do your plugs/wires/coil/cat/fuel filter/Opti/waterpump/O2 sensors have?

Have you had it scoped yet?

It could be a couple of things, maybe getting it dyno-tuned, that may help with the hesitation, getting the fuel/air mixture would be a great thing to dial in. Also, have you ever cleaned out the TB/IAC valve assembly? That would probably help, it CERTAINLY wouldn't hurt.
Changing the plugs/wires once a year wouldn't hurt either
Now was that 30 miles or 30K miles?

It MIGHT be the Opti going out on you.

Have you scanned for codes?
