If the rubber overflow hose from the radiator fill neck to the reservoir has a crack in it, the radiator will never be full. As the radiator heats up and the coolant expands, the pressure is relieved by flowing the excess coolant to the reservoir. When the system cools down, the vacuum created pulls the coolant back out of the reservoir, and into the radiator. With a crack or pinhole in the overflow line, the vacuum is lost.
That would explain the coolant always being a bit low in the radiator, but would not explain a very large/frequent loss.
