There are codes that do not turn on the SES light. DTC 16 for the Opti would be an example. OBD-I has no misfire detection, so a misfire will not cause the SES light to flash. There is nothing in OBD-I to check the high voltage section of the ignition, so problems like burned wires, faulty plugs or a failing Opti cap/rotor will not set a code. An electrically faulty injector would set DTC 18. A plugged or badly leaking injector would not set any code.
If you let it run long enough with a misfire, you may get an excessive rich condition on that bank of the engine, possibly enough to set the code for "lean" O2 readings.
