Car has died 3 times

It is 10:00 and I am on break for 15 so I thought I would ask. I own a 97 Z. Plugs and MSD Wires are less than 6 months old. Recently, the car at idle with the brake on will just die. The idle gets so low it just goes off.
Now this has only occurred three times. Once the car was driven over 15 miles then it occurred when I stopped, the other times I drove a few miles before it occurred. It does not occur when I start the car up and let it idle before I put it in drive to head off for work in the morning of when I go to leave work in the afternoon. It has occurred only at critical times. Two lane road, no turn lanes turning. So any ideas or suggestions. Now I do not notice any other misses or stumbles, but unless it is obvious I would not notice anyway. It has to be very obvious like my car shutting off for me to notice it.

A while back I had a SES light due to a weak voltage reading on the rear o2 sensor, but since I cleared it is has not come back. I store the car in a garage and have no internal mods. My mods are:

G2 CAI with K&N filter
MSD Wires
NGK Plugs
BMR Strut Tower Brace
SLP IAC Sensor
Manual Fan Switch
Jet Air Foil

I would like to add I have 83K on the clock.

rip in elbow,
loose vacuum lines,
loose connections in intake,

I have noticed a tear in my bottom part of the elbow where ther donkey dick as people refer to it goes... Not a huge hole though I would say 1/8th wide 2 inch long roughly.. But a new elbow is like 50 bucks... I will try taping it first then if it works order one. My luck the tape will suck in...

air leak will do it every time!!
