Anyone know PN for Opti-elimination cover?

Anyone know the part number for the GM Opti-spark elimination cover?

According to the GM Goodwrench site...

"This cover / plug is used to cover the hole on the front cover of a 1996 LT4 engine when the distributor is removed and a rear mounted distributor is used."

Was it welded in place? If not, do you remember the size and type of freeze plug used?
