Closed loop permissives

What conditions must be met before the engine goes into closed loop?

I've never found a real answer for this with a set finite list as the code in the PCM would be.
I do however know of several variables which must be met.
Temperature of things like coolant which are in the constants tables
Also there is a timer from vehicle start, so if your car is already warmed up, it will stay in open loop mode for a number of seconds even though the temperature criteria are met.
