What are the symptoms of a bad clutch?

What are the symptoms of a bad clutch?

well i drove my car home to IL from DC for a little vacation and i belive my clutch is going out. I would really like to drive it back to DC and get it fixed back there because i have a mechanic that i really trust that does good work.

Heres whats going on. Its not slipping to the point that, if you give it alot of gas the rpm's shoot up. What happens is if i am in a higher gear, say 3-6 and at a low rpm and give it alot of gas without downshifting the whole car jerks forward. the rpm's dont shoot up it just jerks up to about 2500 rpm. From there its fine no problem, if i down shift its perfectly fine. When i really get on it, its fine, doesnt jerk at all. Another thing that it does is if i am coming up to a stop sign and am at like 25mph and put it into 1st ( not using the engine to slow down just getting ready to take off again) it will grind.
With these symptoms would you say that i would be safe to drive 750 miles home to DC? I already have a SPEC Stage 2 clutch here, but like i said i would rather have it installed when i get back. And as far as the problems, the car runs and drives great as long as i dont give it gas in lower rpms.
Oh one more question, what kind of Gas milage to you all get with minor mods? i'm getting about 200 to a tank. I dot an equal amount of city and highway driving, and rarley use 6th. When i was driving home i got about 320 to a tank, that was all in 6th. Is this really bad or normal?
