Slave cylinder question
Yes, the slave cylinder can be replaced without removing the clutch, but you will have to remove the transmission since the slave cylinder slides over the input shaft. While you are replacing the slave cylinder ,I highly recomend buying a remote bleeder line for the slave so you can bleed the clutch from wherever you decide to mount the bleeder instead of fighting to bleed the clutch from the stock location.

Does somebody make/sell one, or is this like a custom/home fab job??? 
As far as changing the slave cylinder, it's actually "easy" ............ once you get the transmission pulled off the engine

Theres someone on LS1Tech who makes/sells them for 80$ or so. Its braided steel line that goes up and sits next to your master res. VERY cleaver setup, I'll be picking one up come clutch time.
