What hose is this and where can I get it?

Did you read the link i referred to above? The original poster had the exact prob you do..
Get a Dremel, cut off the swaged clamp, and get 5/8" 'generic' hose elbow in any auto store[it's common] and 2 hose clamps..
The hoses are available from GM seperately, but you still have to cut the clamp off.. If you must have the GM part, it's p/n 10215999 'heater outlet hose front elbow'
Of course you could buy the entire assembly, [which is an absolute pain to change] GM p/n shown in shoebox pic..
Another option is the Goodyear Hi Miler kit, for 95-7, that has every hose in it [EXCEPT hose # 15 in the shoebox' diagram is missing from the kit]about 110$ Can be done on the car but it's a REAL pain to cut all the swaged clamps off the hard pipes...
