rocker arm fell off spring
The rocker was loose and it slipped off. The end of the rocker is self aligning, thats the purpose of those two small raised ridges to keep it on the valve.
Look at the top of the valve stem and make very sure it is still flat and NOT rounded over.
This happened to me with a roller rocker and the valve stem was damaged and had to be replaced.

the push rod looks strait and the valve stem looks flat...
i ran it for a bit like that...
do you think i could just try to put it back on and adjust all of the rr's?

You see the allen screws in the center? Those are poly locks. After adjusting the rocker nut the poly lock is then tightened down to keep the adjusting nut from backing off.

when you put it back together pay close attention to the motor when you go WideOpenThrottle.
Usually, when a rocker falls off it's due to the engine being in detonation....(most call it rattle). A bit too lean or too much timing when at WOT will make the motor rattle (overly aggressive PCM tune). Oil pump pickups, oil pump drive gears, broken valve springs, rockers falling off, that's all the indication of rattle.
At least that's been the case with my junk.
