I'll bet your vacuum was the culprit. If you did not have them routed correctly, excess vacuum would try to collapse the innards of the opti. If there was no vent, then there would be excess vacuum. If the vent was connected behind the TB, there would also be additional and therefore excess vacuum.

what do you do with the vacume line in the elbow if you go with forced induction? Would you install a check valve?
