How Does Pinion Become Loose!?!?

How Does Pinion Become Loose!?!?

Hey all,
I was driving my car on friday night, pretty soon i hear a loud whine noise as if my bearings went out... I pull over there is differential fluid everywhere... Im half a block from home so i decide to drive it home... By the time i get home there is a loud grinding noise and a RR-RR-RR-RR sound... i get it towed to my dad's garage... he takes it apart today... turns out the pinion was loose and let all the fluid leak out... and the pinion marked the carrier! How did this happen!?!? i didnt modify it in any way!

Natural wear and tear and possibly abuse. **** happens unfortunately =/.

Same thing happened to my brothers rear a few weeks ago.
It got loose 2 times.

He replaced the carrier since it got scored up pretty bad. He might have changed the crush collar as well.
He put some locktite on the pinion nut and it hasn't backed off since.
