I was doing some searching because I have this weird lean problem. I have been looking at vacuum leaks and fuel press/injector problems. But then I found some post where I could have a misfire issue also. They said it could go lean if it misfires because the O2 had not been burned.
So…… I could have my lean problem if:
1) The spark is good and have a vacuum/fuel problem
2) The vacuum and fuel are good but there is a spark problem
Basically anything could be my problem.
Is that correct or did I miss something?

You probably read this in your searches, but here's the cliff notes:
Ignition problems can cause a FALSE lean condition. A misfire sends unburned fuel and air out the exhaust. The O2 sensors read the extra air and it makes them THINK you are runnign lean, even if you are not. So it adds fuel. Which makes the engine run RICH, at least on the bank with the misfire. BLMs will be too high (well above 128, up to 160) when this is happening.
Vacuum leaks and fuel starving cause ACTUAL lean conditions. Too much air, too little fuel, respectively. Again, the PCM responds by adding fuel, and the BLMs will be high as a result.
So no, not everything causes lean operation. Running a search for 'lean' will, however, find posts with all the causes you listed, as they all involve real or false lean conditions, and high BLMs.
