Throwing belts and chirping noise

My serpentine belt has walked on me twice in the past three weeks. First time was during autox, the second time during normal driving. I just installed the ASP crank pulley last December. I have a mild chirping noise as well.

I wouldn't be surprised if the belt is a little fubar from riding on the edge of the upper pulley's. Should I look at replacing the tensioner and/or belts? Crank bolt loose?

Waddya think?

Id reinstall the stock crank pulley and drive it around checking to see a difference. Obviously if your problems go away, the aftermarket pulley was the problem. If the problems remain after reinstalling the crank pulley check the alignment of your other pulleys especially the power steering pulley since its adjustable.
