Slave Installation - NEED Help ASAP

Slave Installation - NEED Help ASAP
OK..... car has been down for over a year and I forgot how the slave cylinder looked when properly installed on the bell housing.
I was able to install the slave over the two rods on the bell housing while making sure the rod on the slave was inserted into the socket on the clutch fork. I then put a nut on each rod to hold the slave onto the rods.
My question/s..... should there be nuts installed first before the slave is put on the rods used for jam / backing nuts??
How far do I run the nuts down on the rods??..... so far I only have then down about a 1/4"
When I depress the clutch pedal, there is movement of the clutch fork but the slave appears to be very unstable.
Any information and pictures would be greatly appreciated on how to properly install the slave cylinder.
Thanks!!

Appears I'm missing the spacer between the slave and bell housing.... does anyone know what part number this is and where I can find one??
I tore apart my garage trying to fine it..... it either did not have the spacer when I took it off.... or I lost it somehow??
