grinding with clutch fully engaged?

grinding with clutch fully engaged?

I've noticed this for awhile now, and it's really pissed me off. Every once and a while, I've gone to put the car in 1st to take off with the clutch engaged, and grinded going into 1st. It just happened going from the bank drive through window. I was in neutral and went to put it in 1st, and grinded 1st. Also, if I'm slowing down, I also can't seem to put it in 1st above around 20mph without grinding a number of times either. I know I'm pushing the clutch all the way down and not part way. What's the deal? The transmission shifts fine otherwise, but this has happened a number of times, and I keep thinking I'm gonna destroy the synchros in short order with this. Anyone else have anything similar to this?
