driviing car shifted it hard 1-2, now problem

now the clutch fork is hitting he pressure plate...i pulled tranny real quick looked it looked fine i thought a bubble in the slave was the cause...put it back together and its still doing it...

the fork almost hits without eh slave on it at all...what is probably the cause?

i read to grind down the t-bolt but thing is it was fine for 100's pf miles before...only thing i can think is the fork bent...but i pulled it off the tob and looked through the hole and it appears straight????
