When I start the car, I can't go in reverse, and it doesn't want to go into any other gear unless you nearly force it. You can actually see the car start to roll forward as I try to put it in gear with the clutch pedal to the floor. I can put it in reverse with the engine off and start it that way. It shifts very hard once on the road, and forget about downshifting unless you rev-match the engine before attemting to downshift.
Fluid reservoir is serviced properly, and the clutch pedal feels firm, which would tell me there doesn't seem to be air in the lines. The clutch was replaced about 80K ago.
Would this be the throwout bearing, pilot bushing holding on to the input shaft, or pp spring problems? It did this (not as bad) about a year ago in a traffic jam, where I had to slip the clutch continually for about an hour, but went away slowly over a weeks period. I thought the pilot bushing and input shaft might have held on to each other do to the extreme friction heat.
Ideas?

sounds like a faulty realease bearing aka throwout bearing. since the input shaft only moves in rotational directions, the pilot bearing cannot be the problem. if the pilot bearing was stuck on the input shaft, it'd just spin in the back of the block/crank assembly. you could also possibly have something wrong with the pressure plate, but that would not be nearly as common as throwout bearing problems. I'd put my money on the throwout bearing, but you'll easily see the problem once it's apart.

Sounds like the disc isn't disengaging fully from the flywheel. Could be the springs in the center of the disc. I've had it happen twice.
