2 Days ago I had a roller rocker come off of the push rod recently after a fresh re-build. It damaged the guides so I bought another one. Installed it by bumping the starter untill push rod sat at the bottom of the cam, did the pre load as directed (1/4 turn), drove the car about 6 blocks and it came off again. 1.52 comp roller rockers and comp lifters. WTH!! What would cause this?

You didn't reuse the push rod did you? Roll the rod on glass, even a tiny bend will cause this. Happened to me before, just got a new push rod at autozone and has been fine since...

Yes I used the same push rod, it was ok, plus they were brand new hardened push rods when it fell off the first time.
They don't have poly locks they are the lock nut style, and I tightened them down to adjust lash.
There isn't a flat spot on the rocker guide its just round and these are the same rockes I have been running even before the build.
Any other ideas. I'm gonna try and bend the guide back and try it again.

What exactly bent that you're planning on bending back?
You can't "bend" a valve guide - it's located inside the cylinder head. Clear up your terminology a bit. The valve guide is what the valve stem passes through from the combustion chamber through the valve seal.
Perhaps the suspect rocker's alignment tabs are worn off. Take a look at the tips.

My bad on the terminology, I meant to say I bent the rocker alignment tabs back cause they bent a little when things came apart. The rocker was new prior to falling off so they we good before. So, I put them on again and same result. I think I need to check to make sure I don't have a broke spring and also triple check that both intake and exhaust push rods are on bottom of lobe before I set the lash. Sound about right or any other ideas?

make sure you are adjusting the valves correctly, here is the method i use:
(E-O-I-C) method : Exhaust-Opening/Intake-Closing. when the exhaust valve starts to open adjust the intake valve on that cyl. when adjusting for the exhaust valve watch the intake valve open then on its closing cycle, about 1/3-1/2 max returning, then adjust the exhaust valve on the same cyl. this method you don't have to go through any firing order you can just start at one end and work your way around.
