Grinding gears above 3,500 RPM shifts (manual tranny)

I have recently noticed that if i rev out in any gear and attempt to shift, I feel a sudden grind that isn't exactly friendly. It used to happen every once in a blue moon so I swapped out the tranny fluid for Honda ATF. The grinds went away for awhile and now they are back and happen more often. I know I need to change the rear diff fluid but I don't think it would have anything to do with it. Could it be that my clutch is going south or are there bigger problems with my T-56 that I'm not seeing?

Maybe you just suck at shifting. Sorry I just couldn't resist. Sounds like maybe bad syncros

haha.. i most likely do... but yea I guess that could be the culprit. For the time being, I've resorted to what some people would call "double clutching"clutch in, pull out of gear to neutral, half a second, then pop it into gear. There is absolutely no grinding through this method, it is just annoying.
