installed F.A.S.T. no start!!

I installed my FAST bank-to-bank with wideband the other night and could not get it to start. The best it would do is fire up and as soon as it fired up it would immediately die. I wired everything up to my stock computer very methodically and there is no way that i could have missed a splice or had one grounding out etc...
Possibilities:

1. There was ONE wire. on the third step of the instuctions it said to splice BLK/WHT wire from FAST computer into BLK/WHT wire in GM PCM A(red connector) cavity 18. The only colore wire in A 18 was a TAN/WHT wire. The rest of the instuctions were perfect. I tried starting it with it connected to A 18 and without connected. No difference.

2. I left my O2 sensor unpluged. I know that it should be connected but i didn't have time to do it yet and just becasue my o2 isn't hooked up dosn't meen that the car shouldnt start...
any thoughts???

I ALSO GOT A --->P1351 IC HIGH VOLTAGE CODE.
Why would i get this code?? Auto tap only read 13.5 volts coming from the sensor...

Thanks for the reply guys, but i am still not sure what causes this trouble code...I have a delteq system that has worked fine for a while, unless something on that went bad??? Unlikely though.
so what things cause a p1351 code??
