The most common cause is high fuel pressure caused by a failed fuel pressure regulator, or it has been disconnected from manifold vacuum.

where can you find the failed fuel pressure regulator?

Under the TPI plenum at the back on TPI equipped cars.

I usually see bad or marginally bad O2 sensors, cause it too, especially TBI for some reason.(Like my truck for instance..)
Also check plugs and plug wires, distributor cap and rotor..weak ignition can cause it too..:)
Carbed car can be dirty air filter, needs tune up, idle mixture wrong(turn screws to get the highest idle speed, on feeback carb its an electric device like an IAC but for fuel), O2 if equiped..
