Stuck in cell 16

Well I changed the injectors in my car and reprogrammed the pcm for them. After getting home and changing the plugs and a O2 sensor the car is stuck in Cell 16. Also one of the O2's is stuck reading 449. I am not sure why but I put a new O2 in it and it did not fix the problem.

Any Ideas
